Direct Care Staff

Job Description

Clinical Care

  1. Responsible for the effective management of ward with quality service programme in accordance with the organisation’s QA and Infection Control programs.
  2. Responsible for the supervision, evaluation and disciplining of all staff.
  3. Responsible for upholding the policies and values of the organisation and the nursing profession.
  4. Co-ordinate teamwork in the day-to-day running of the nursing department.
  5. Assist the DON in evaluating and maintaining a high standard of nursing care through clinical guidance, teaching, support and advice to nursing cum auxiliary staff.
  6. Orientate all RNs and appraise RN’s or SN’s clinical assessment before their confirmation.
  7. Ensure that effective and cooperative working relationships are maintained within the Nursing Department and between internal and external customer groups.

 

Quality

  1. Review and update nursing policies and procedures.
  2. Ensure team members are familiar with the Standard Operating Procedures (SOP) and all emergency protocols.
  3. Lead quality assurance activities to monitor and evaluate the quality and appropriateness of nursing care, and implement action plan to close gaps identified or for improvements. Ensure quality audits are planned and conducted periodically as mandated by the Ministry of Health.
  4. Work with nursing team, allied health professionals and support care team to coordinate, conduct and facilitate quality improvement initiatives and continuous improvement projects.
  5. Maintain systematic inventory control of assets and drugs to achieve optimal utilisation and safe storage.
  6. Coordinate, manage and evaluate the services of the ward to ensure the provision of safe, effective and quality care to residents.

 

Training and Development

  1. Direct and facilitate staff development, skills and competency upgrading through various opportunities.
  2. Assist in regular and on-going assessment of nursing staff competencies to ensure achievement of targets set and/or expected competency levels.
  3. Orientate and appraise newly joined staff nurses clinical performance during their induction.
  4. Assist the Director of Nursing (DON) in identifying, planning and making decisions to meet training and developmental needs of nursing and ancillary staff.

 

Administration

  1. Ensure appropriate skills mix and adequate manpower resourcing for all shifts and types of services.
  2. Utilise performance management and counselling skills effectively to motivate staff to perform at their optimal level and ensure care delivered is in compliance to standards of practice.
  3. Ensure the safe storage of all confidential files, records, documents; and that residents confidentiality is maintained at all times.
  4. Ensure that effective and cooperative working relationships are maintained within the Nursing Department and between internal and external customer groups.
  5. Maintain an inventory list of ward equipment and ensure regular maintenance is carried.
  6. Support the DON in the management of the Home and deputise in her absence.
 
Job Requirements
  1. Bachelor’s Degree in Nursing with minimum 5 years work experience or Diploma in Nursing with minimum 10 years work experience. At least 3 years in a supervisory or managerial position in nursing home setup preferred
  2. Possesses good organisational, management and problem-solving skills
  3. Strong team player with good communications and interpersonal skills
  4. Registered with the Singapore Nursing Board
Job Description

Clinical Care

  1. Make frequent rounds and attend to residents’ needs. Ensure the safety, comfort and well-being of the residents and report residents’ needs to the nurse
  2. Assist in ensuring the comfort and good personal hygiene of the residents under his/ her care, and keeping them clean, e.g. oral toilet, showering and grooming, trimming of nails, changing of diapers, changing of clothes and bed sheets
  3. Assist in serving meals and oral feeding of resident without swallowing disorders
  4. Assist nursing care staff in the transfer and positioning of non-ambulant residents
  5. Assist and participate in residents’ recreational, occupational, social and therapy programmes
  6. Assist nurses to ensure residents take their medicines
  7. Accompany residents to specialist outpatient clinic for follow-up appointments and outing trips
  8. Assist in last offices

 

Quality

  1. Maintain a safe and clean environment, e.g. cleaning and mopping of spillage or discharges, cleaning of residents’ bed and bedside locker, making of beds

Job Description
  1. Treat resident by using therapeutic and self-care activities designed to improve function under the direction of the occupational therapist
  2. Monitor a resident’s activities to make sure they are performing them correctly and to offer encouragement
  3. Contribute to meetings and case conferences to ensure coordinated and comprehensive care plans for patients
  4. Teaches and guides residents on how to use equipment that may make certain activities easier
  5. Document resident’s daily progress in appropriate records
  6. Maintain the therapy areas, equipment, and supply inventory regularly

Job Specification

  7. Prepare and administer medications (oral and subcutaneous) to residents, complying with safe medication practices (only for those who have been assessed competent in the administration of oral and subcutaneous medicine)
  8. Supervise junior staff to ensure safe and quality resident care, and in compliance to standards
  9. Deal with emergency and incident that arises and ensure prompt action to mitigate situation. Escalate promptly to staff nurse/ supervisor, where necessary

Conduct comprehensive psychosocial assessments, including coping, strengths, risks, and support systems. Take lead responsibility for monitoring psychosocial well-being within assigned wards.

Counseling
Provide counselling and psychosocial interventions to residents and families with identified needs.

Facilitation
Lead family conferences (e.g. Advance Care Planning, care discussions, financial matters) and manage complex family dynamics.

Risk Assessment
Identify, assess, and manage risks (e.g. self-harm, behavioural concerns, family-related risks), exercising professional judgement and accountability.

FinancialAssessment & Assistance
Conduct financial assessments and guide applications for assistance schemes(e.g. Medifund, MFEC, AIC LTCS), including complex cases.

Programme Implementation
Support departmental initiatives, resident engagement activities, and contribute to service improvements.

Discharge Planning
Lead discharge planning in collaboration with multidisciplinary teams, ensuring safe and appropriate transitions of care.

Information & Referral
Independently assess and link residents and caregivers with suitable community resources.

Documentation & Reporting
Maintain clear, professional, and accountable case documentation and reports.

Job Requirements

Educational level

Degree in Social Work/Graduate Diploma in Social Work

Experience level

  • Experience in case work, client engagement, or social service delivery
  • Demonstrated ability to conduct psychosocial assessments and develop and implement intervention plans independently.
  • Experience managing complex psychosocial cases is an advantage
  • Familiarity with community resources and inter-agency coordination

General Requirements
